Rock Auto just sent out this list of closeout items for 4th Gens. They have the exact fit rear wiper blade refills again, they're $1.26 each.

Can't give you a timeframe but when mine starts smearing instead of wiping, I clean it and that helps. When it stops helping, I replace it. I've had my 4Runner for 3+ years and replaced it once so far. Having a couple of spare blade is probably close to a lifetime supply.
