wheel lug stud spinning and spinning

I got the lug stud and lug nut off last night. I drilled through the lug nut into the stud. First I drilled straight through the lug nut key with multiple bit sizes until I had a 1/2in hole through it. I then held the lug nut/ stud from spinning by holding the key with vise grips while drilling into the lug stud. I started small and worked my way up until the stud was close to paper thin, then I removed the other lug nuts and grabbed the wheel and snapped the problem lug off.

Looking at the old stud and everything I cant determine why it happened. I can only speculate that the lug was cross threaded and got stuck then I hit it with an impact gun to tighten it. But even thats hard for me to believe cause I start all the lugs by hand with the lug nut key. So its just one of those things Ill never know why it happened.
