    Any telescoping ladder will work and then I'd recommend fabbing up some sort of bracket that attaches or provides a stand-off from the rack. I currently use a ladder with my RTT which is maybe 1.5-2" proud of the side of my rack without problem.

    I've cut off LCA on 4Runners before, we usually use a combination of 6" grinder disk, sawzall, and air hammer to get in there between the bushing and brackets. If you nick something a welder helps, but I know not everyone has those around.
