did u install new pads ? if not than the old pads are warn to the old rotors. you need to replace the pads and cut the rotors again. if you answered yes to the first question then take it to a shop who can turn the rotors on the car. on-car lathes are much more precise than standard brake...

sounds like a balance issue, took me several times to to get the shake out of my cooper stt's, although last year my ball joints were sloppy which felt like a wheel out of balance. you fill it more with offroad tires or a/m wheels.
