85 4-runner rear coolant hose leak

On my 1985 4-Runner (same heater/coolant piping setup as 84-95 4-Runners) about 2 months ago all the sudden one of the two rear (back seat) heater hoses underneath the truck began to leak coolant at the end of the rubber hose towards the steel hose about one drop every 4 hours so I decided to loosen the Tridon hose clamp and position it more towards the end of the rubber hose (towards the steel hose) but it's still leaking.
How do I go about fixing the problem?.Thanks in advance.

replace the hose and clean off the pipe that the connects to the hose.

Anyone know the exact rubber hose specs for the rear heater coolant pipes? so that I can go to my local auto parts store and buy them by the foot.Thanks.
