Is it the rust ring/ridge on the rotor that is stopping you? If so then you must squeeze the pistons back into the cylinders to make some room.

I haven't changed the pads on the Runner yet, but it looks like the same design as in all Toyotas I've owned. So I don't think there would be anything really holding the pads on the caliper. There may be a spring that pulls the pads off the rotors, but that is very small and doesn't really hold the pads in place.
Maybe you just need to wack the caliper with a rubber mallet to loosen it up. If no rubber mallet, a piece of wood on and a regular hammer work tool.

I recently replaced the front pads on my '03 Runner. I fouind it not too difficult.
These front calipers are a 4 piston type that is bolted directly to the suspension arm. There are 4 pins(2 per caliper) held in place by small wire clips. Remove these first. The pins on my 4Runner were very difficult to get out. Corrosion, salt from winter driving etc causes these pins to freeze in place. With a bit of peruasion from a mallet and a drift pin, I managed to get these out.
I too had problems getting the pads out so removed the whole caliper assembly (2 bolts - retorque to 90 Ft Lbs) and everything came out together.
Be sure if you remove the caliper that you put a couple of lug nuts on to hold the rotor in place before you remove the caliper assembly or the rotor will slip out of alignment making it difficult to get everything back to together.
I used genuine Toyota parts and I see from previous posts that is your best bet.
I have ordered some new pins and will replace them later since the original ones are badly corroded.
I recommend that if you drive on snow covered salty roads in the winter that you replace these pins with every brake change. give the calipers a good cleaning as well and with a wire brush and clean out any corrosion that may hinder proper brake pad movement.
As I said. it's an easy job but take your time.

I just changed my brake front pads. I bought the pads at kragen for $60 ceramic. Took me about 1.5 hours, don't have to take out the brake caliper. First of all, need to remove the the little spring that hold the long thin rod across the caliper. Then use a flat screwdriver to pry loose the brake pads. Once the pads are off, then use a C clamp to clamp down the four pistons. Don't need to bleed the brake fluid. Just make sure to step down the brake paddle a few time. My next job is to change my plugs. I just ordered some NGK iridium from the internet.
