Brake help!!!!! Any info please

I have a 2004 SR5 with a V6. It has 145000 miles. About three weeks ago I lost my brakes. High pitched noise from under the dash and couldn't press brake pedal. Dealer said it was the master cylinder/actuator. Had the unit replaced and the brakes have been working great until this morning. This morning when I started my truck it happened again. I plugged in a code reader and the OBD is not showing any faults or bad codes. Dealer had said it had tripped two codes the first time. I know these master cylinders have had some issues but a new unit shouldn't have failed this fast. The high pitched noise actually originates up under the dash and not from the master cylinder. Something else has to be causing this. Has anyone experienced this themselves or have some knowledge. Any info is greatly appreciated.

ODBII reader won't show these codes all the time, I'd go back to the dealer, should have warranty. I just had mine replaced recently too.

EDIT: Pretty sure you need an ABS code reader to she the codes.
