Car horn not working

Well, I got a new clock spring on eBay for $50 that appears to be genuine (came in Toyota/Lexus packaging and everything). Got it installed and the horn honks now! Just figured I'd post this for anyone searching in the future.

I had to transfer the steering angle sensor over but that was trivial to do. I wonder if a new sensor comes with the spring when you buy from the dealer... that would explain their exorbitant $500 price!
 
5th gen Limited has extra unused wires in the clockspring.

If you have ruled out everything but the clock-spring, check out Partially failed Clockspring workaround | PriusChat , my 5th gen 4runr limited has the same setup. I was able to move the gray wire over one pin on the 12-pin connector (on the column) and move the blue wire inside the steering wheel from the 10-pin connector to an unused pin on the 4-pin connector. Horn is working again. I used a hobby electronics jumper to verify which pins are connected on each side of the clock-spring with my meter.

Be careful with the airbag, but no need to remove steering wheel. Re-pining is not fun but doable.

Horn not working when steering wheel straight

My 2016 4runner has a problem. Think I can re-create it anytime. If steering wheel is straight ahead, horn doesn't work, if I move steering wheel 15-20 Degrees, maybe less in either direction, horn works.
Is that a sign of the clock spring? Or???

Yes, that would indicate the clock spring.
 
Anyone have any definitive way to diagnose it's the clock spring.
A day or so after I noticed my horn not working, my back up camera screen also didn't work about 3 times.
Hasn't been a problem since, not sure it that's related or not, but want to make sure it's good before I tear my steering wheel off, well remove it gently so the airbag doesn't go off, I'm a little scared of it. :)

- I replaced the 10A horn fuse
- I am reading 12volts between + battery terminal and inside metal bracing of steering wheel. When I touch the ground wire that is attached to the back of the horn button inside the steering wheel, without depressing the horn, I get 0V. When I touch the ground wire inside the steering wheel when depressing the horn I get 12V.
- I get 12V between + battery terminal and ground location of horns
- All other functions like cruise, vol up/down etc work on steering wheel

Any thoughts would be appreciated!

Does the horn start to work when the steering wheel is rotated through its range of motion? Try rotating in both directions starting from straight with the horn held down the entire time. As many others including myself have found this is a strong indicator of damage to the spiral cables which needs the clock spring assembly replaced.

I tried this the other day and no, it doesn't work at any steering wheel angle. I know its not a definitive indication that the clock spring isnt broken but all other buttons on the wheel are working.

The fact I'm getting 12v between the steering wheel horn ground cable and battery positive when I depress the horn makes me wonder whether the issue is in the "K Unit" integration relay?
 
Pull the connector off the horn and connect a meter between that connector and ground. Push the horn pad and see if you get 12V DC at that connector. If you do, then the horn id bad. If you don't then you'll have to troubleshoot back. Also did you check the fuse with a meter? Sometimes a blown fuse can look good, but be bad.

About a month ago I have replaced clock spring assembly to resolve horn issue on my 4Runner. In my case, only horn was impacted, all other steering wheel buttons were working.
Did you check if horn work using panic button on the key fob? if horn works in panic mode then I would say with 99% certainty it is spiral cable (clock spring) issue.
