Clutch release bearing issues.

So with all the crazy information rolling around in my head I am having a heck of a time sorting out what could be causing my throwout bearing on the TDI swap to go bad instantly.
I attached a few pictures for you guys to see the damage caused by the bearings, any insight would be helpful.

i assume the bearing fits over the input collar, rather than binds tight to it and thus not spin?

Yes it's a free floating unit so the only way I see it slamming the clutch tongues like​ that is if the slave cylinder didn't let it return to home position but that's not possible as it's a simple lever. I'm stumped I'll be putting a new bearing in tomorrow morning and I'll grease the shafts and pivot points good but I'm at a loss to what caused it.

how about depth? is the bearing being jammed to the back of the collar because there isn't any clearance between the clutch and transmission?

I just have to ask: you don't drive with your foot resting on the clutch pedal right?
To cause that damage, something ISNT spinning at some point. Must have made a terrible racket huh?

Did you confirm the required play between thr pedal and master's plunger? Did aomething change there?

That quill (hollow shaft) sticking out of the transmission looks to be in fairly rough shape. The TB slides on that, and I doubt it's sliding freely with that much corrosion, dirt, and rust on it. I would clean that up down to bright metal and make sure the TB slides freely all the way to the back.

We have a winner I think. After cleaning up the shaft and putting a little grease on the inner side of the new throwout bearing careful install it is silent and working perfectly.
