Cold Start Idle Issue

I did some hunting around on this forum and couldn't seem to find anything that quite fit my situation. My GF has a 1999 3.4 4Runner that has recently started having some idle issues. On cold start her truck will idle very rough for a few seconds but goes away very shortly. It will occur in park or in gear. Lately it happens every third day or so: it's not consistent. Her truck has around 135K with about 20K on a Toyota installed long block. It hasn't seen any recent changes.

I'm going to start with the basics and check fuel/spark/air. Reading on the forum it doesn't sound like a MAF issue as the problem isn't happening all the time. Any thoughts?

Does the 3.4L have an Idle Air Control Valve? If so, then it sounds like you need to pull it and clean the plunger. They can get a heavy carbon buildup and cause the plunger to stick...

Mine started doing that a few weeks before the TPS went bad. Relpaced it and it has never been back. I would meter out that Coolant Temperature Sensor as well. That sensor takes the place of a choke on the old carberated engines. It tells the ECU how much fuel to deliver at various engine temps.
