The actual deadline date is May 31, 2017, but I would recommend to not wait and act fast so you get in line because the sooner you get the dash ordered the sooner you will get it replaced because of the long wait for the dealer to get the dash.
"The Primary Coverage offers warranty enhancement described herein for all owners of covered
vehicles until May 31, 2017, regardless of mileage or date of first use of the vehicle."

Taking mine in tommorow for the dash replacement!!

Because of them having to take apart almost everything in the front interior except the seats to replace the dash I also did a video documenting that there's no warning lights on, no tears, scratches, etc. in the interior, showing that everything works as it should, also a walk around the exterior, just wanted to have proof that the T4R was in great condition if my T4R comes back with any issues, better safe than sorry!!

Dropping mine off Saturday morning. Been waiting since June 2015. Long wait, but thrilled it's getting replaced!

The dash replacement went very smoothly, the Tech did a great job, I couldn't find a single issue after the work was done, the only way I could tell any work was done was by seeing the beautiful new dash and he was very fast, I think he may have set a record, 2.2 hours, most may have fretted that it was done so fast but I attribute it too he has done a lot of these dashes, I was told he and one other guy are the only Techs they let do these dashes because of the care they take and not to mention the speed of completion of the job.
I think another record was set on how long I had to wait for the dash to get to the dealer and them getting it installed, from the day I took it in for inspection until today was 2 months and 3 weeks, 4 days of that wait was because I couldn't take the T4R in immediately after them calling me, so in reality it was 2 months, 2 weeks and 3 days, I was so shocked but very pleasantly surprised when they called and told me the dash was in!!

I thought my '05 had escaped the cracking dash until yesterday when I was cleaning the interior I noticed a fine, 2" crack starting. I called Toyota and they told me to bring it in for documentation which they will submit to Toyota. I'm hoping it is approved. I have had this vehicle for two years.

It's good that it starting cracking now instead of 3 1/2 months from now or you would miss the deadline to get it documented for getting it replaced, so you're unlucky and lucky at the same time!

Mine failed the inspection... even though my s/n fell in the range, they couldn't find anything wrong with it and said it was fine.
If it starts cracking on June 2, I am going to be irate.

Maybe take it to another dealer, or somehow maybe a mysterious crack/s could appear any day now, try to flex/pushing at the padded sections often for the next 2+ months hint, hint.

I have two 2004 Limited 4Runners (one is my sons). I took mine to dealer A and they hardly looked at it, it never left the service bay. They just glanced in the direction of the dash, wrote me up and sent me on my way.
I took my sons to dealer B and they had me wait an hour and a half while they inspected it in the shop, took photos and everything. They approved it but my point is some dealers are more lax about approving the dash replacement.

The install on the 4th went beautifully. Took about 2 hrs. 21 months of waiting, but it makes the interior look almost brand new. Granted, 2004 brand new, but still... Now let's see if I can get my wife's blessing on changing out the JBL head unit...
