Crankshaft pulley removed itsself

So I was driving down the road tonight and my battery light comes on,no biggie i though maybe the belt broke again or the plug came loose. Well I keep driving until I get to the restaraunt and pop the hood grab a flashlight to check to see if the belt is missing and the crankshaft pulleys are stuck in the radiator. No noise no nothing, i was just thinking like wtf just happened here.

Has anyone else had this happen to them and whats the best fix to prevent it from happening again, and what made this happen if anyone knows.

When was your timing belt changed? How many miles on the motor?

Timing belt was changed at 160k miles before i got it, and it has 230k miles on it now.
