Anyone modded their cruise control to raise their idle a couple hundred RPM while wheeling? Probably more suited to the crawling guys driving a manual. I haven't yet looked at the wiring or controller for it but bypassing the clutch and brake switch shouldn't be too hard, not to sure about the 50km/h speed requirement though.

My buddy did it on his 86 Runner- he used a shifter from an old road bike to regulate engine speed and hold it. Uses cables so 100% reliable (unless the unlikely situation where the cable snaps).

Add a hand throttle with the bicycle brake lever and cable. Like Liny mentioned. People do this all the time. You clamp it to the shifter so you can operate it easily Plus you have an open spot on the throttle body to land the new cable since you dont have the auto.

I figured as many as needed, You can drive with a foot on the clutch and brakes. I have heard some people use it for increasing rpm's for engine driven air compressors, and for hydraulic winches.
As far as putting on in with out a crawl box I dont really see too much use for it

Here is my Mod. Only takes a switch and some wires.
Install a switch in a convenient place in the dash board.
Runs wires from switch to diagnostic port on engine.
Connect one wire to E1
Connect the other wire to TE1
This is good for a 400 to 500 rpm increase at the push of a switch.

gotta be a shift lever- a brake lever will return to 0% once it's let go...the shifter will hold it's position wherever you leave it.

might not be 100% good for the truck to run it with a diag connectors shorted like that...I believe in most repair manuals it says not to drive it, and they rarely (unless it has to to with idle or something) even recommend to have the vehicle running...

I actually have a thumb-throttle like LINY & J$ mentioned - purchased as a kit (for my old FJ40) from Man-a-Fre, I think, years ago. I never installed it, so now I just have to find it and see if it'll fit. 
No reason that it shouldn't work, although I imagine I might need a longer cable.
But basically, IIRC, you mounted the lever portion to your 5spd shifter, conveniently located under your thumb, and thread the cable down through the boot, under the firewall and hook it up to the throttle body. Push the level forward with your thumb and voila, just ignore the big pedal on the right at your feet.
If I can find it, I'll do a write-up as I install it. It should still be in the original box, so I'll hopefully have part numbers ...
