Is it worth it to buy extended factory warranty for my 03 T4R limited. I just receive a mail that's offering the extended warranty.

It depends where you get it from, most of the companies selling extended warranties is a sham, too many exclusions, limitrations, PIAA..:sobbing:

Unless its a Toyota Warranty (and that's usually contracted out anyways) I wouldn't waste your time.

toyota extra care warranty has paid for itself for me with 2 different vehicles. with the runner they replaced 2 cats, on a solara they replaced all engine mounts.

Thanks for your reply. The extended warranty is from a 3rd party company and base from your replies, it's not worth it. My runner powertrain coverage already expired without reaching 100k, so I'll just skip this one then.
