I was under the impression automatic 3rd gens can get 19-20+ MPG pretty easily, as long as maintenance is kept up of course.
I should be able to tell you in a few days what I get, I just bought a 2002 yesterday

Right now I have a 99 SR5 manual tranny with 142K on the odometer. I actually only get between 17-18 whether in the city or on the highway. It's turning 2800 RPM's at 70 MPH. I have owned several automatics and they always averaged 19 MPG combined. I know the stick is supposed to get better mileage but not this one. It has new plugs and wires, new timing Belt and pump and always fresh oil. This is just letting you know that the 5 speeds don't always get better mileage.
I get 80 MPH at 2900 RPM. A/T.
Check your clutch free movement between the pushrod and the clutch arm in your slave cylinder. Do you have a free movement in your clutch paddle.
How old is your clutch... always replace the clutch and the clutch coverwith AISIN

Old school 5 speeds always get better mpg. This is less true on new cars with CVTs, 6+ speed automatics, dual clutches etc...
But in in the case of the 4Runners, you have 5 gears, automatics have 4. You have a clutch. They have a torque converter. Unless you drive like a dong you will always have better efficiency than an automatic.

How does that affect fuel mileage ?
You want to make sure the clutch is not slipping. this can happen when there is no free movement in the clutch pump push rod and the clutch paddle. Same with the slave cylinder push rod and the arm that operate the clutch cover.
Also Aisin makes the OEM toyota parts. I remember when I bought my car, it had bad braking. I replaced all with OEM toyota brakes and trust me with much less force I can slow down my car. The difference is the material in the brake pad. Similarly, the material in the clutch is also important.

I know very little about clutches and slave cylinders, etc. I am not afraid to try anything though, as you know by my recent water pump/TB change. Can you point me to some illustrations to educate myself on the clutch system?

My understanding is both get similar gas mileage.
16-19 is NORMAL.

Is yours a V6? What gearing do you have and what size are your tires? I have a 99 limited 4x4 that turns about 2350 rpm at 70mph with 4.30 gears on 265 75 R16.
I average around 17 city 22-23 on the highway at 70mph (flat roads) and 18-19 mixed

One end of the crank shaft is the timing belt and the other end is the fly wheel (FW). FW has an attached pressure plate (some times called clutch cover). FW is bolted onto the crank shaft. Pressure plate is then bolted on to the FW. FW and pressure plate spins at the same time.
The clutch disk is between the FW and pressure plate and suspended on the input shaft of the gear box. When you press the clutch paddle , it will cause the mechanism in the pressure plate to pull back releasing the pressure on the clutch disk. This will disengage the FW and the clutch plate, hence no power going into the transmission.

I get about 18 or 19 pretty consistantly with a 5 spd. That's mixed driving. I have gotten as high as 21, though, but during the winter with sucky gas I've also gotten 15 or 16.
