Gas mileage problem

I have a 99 4runner sr5. I only get about 200 miles on a full tank! What could be causing this terrible gas mileage? It's got about 118k miles and is in great shape besides the gas issue. Any advice would help, thanks.

Bad plugs, leaking injectors, bad oxygen sensors, broken vacuum lines, dirty filters, clogged injectors, heavy foot, faulty fan clutch, bad wires, fly on a MAF sensor (seen it), sticking throttle body, bad math, low tire pressure, old fluids, frozen brake calipers, superchargers, poorly made air intakes, rock and roll, Honey Boo-Boo, or ****ty quality gas.
 
I vote for the ****ty quality gas we have been forced to use. (Winter blend/10-15% ethanol) Thanks Gubmint....for everything! Ive changed plugs, air filter, cleaned MAF, cleaned Throttle body, new oils in everything and made NO difference in mileage. Truck feels good though. Feels like it will go forever. Just getting there will be really expensive. I would expect a 5-600 HP Corvette or Supra to get this kind of mileage 15-16.. Really???!!! I have a 93 Civic that used to get 36 mpg, after all the Gubmint regs on fuel, i cant get over 31 in it now. WTH?
