Headlight Issue

So I have been having an issue with my headlights and fog lights lately. First my headlights dont want to turn on everytime I flip them on. I have to flip the switch multiple times for them to turn on. Second, when I turn my lights off my fog lights stay on for 5 seconds or so.

I was thinking maybe its a relay issue or something but dont even know where to start.

please list all headlight mods you have . like hids , also is there a relay harness? hid brand? check the fuses and connectors.
