i don't have a diagram, but it's pretty straight forward...
out of the top of the caliper theres a little notch on the break pads that you can squeeze against the caliper to compress the piston a little bit, then you can take out the 2, 17mm bolts from behind the remove the caliper. from here you can slide the rotor right off (might be tough, you're replacing it anyways so hit the face of the rotor a couple times with a nice sized hammer, it'll come off). make sure you clean the hub (where the rotor sits) well with a wire brush or something so no rust comes between the rotor and the hub. then you can finish compressing the caliper and remove the pads...the pads have two pins that go through the caliper and hold them in place, and they're held in by a long spring. take this spring out first, then tap the pins out (thy might be a little tough, hit 'em with WD40). now you can put the new pads in and just reverse the order! and you're done! hope that helps
