Hitch/Tongus/Ball Question

I have a '05 V8 4R.
Like most others, it came with the little box with the tongue and reducer/bushing in it.
I finally went out and got a ball to attach to the tongue (think thats what its called). Anyway, when I put the reducer/bushing in the tongue hole, install the ball and tighten the nut all the way down, the ball will still rotate. Now the ball is not going to come loose, the nut is tight. But again, the ball will still rotate.
Is everyone elses like this? Is this safe? Is it supposed to be like that?
I don't really have any experience with hitches so I'm not sure if this is corrrect.

Sounds like some are grinding down the bushing, but most are getting a ball with a 1 1/4 shaft.
 
The ball I purchased has a big lock washer that went under the nut. It keeps the nut from backing off and keeps the ball from turning.

Does it have a 1" shaft or a 1 1/4" shaft?
Seems to me if it is a 1" shaft that it will 'give' or 'move' when you go to tow a trailer even though it's tight. Not a good situation IMO.

I'm going to look for one with a 1 1/4" shaft and take the one I bought with a 1" shaft back.
