start by removing the lift gate panel on the inside of the door. there are those lame dealer clips holding it on. the take the top window rubber thing off. if you look inside the panel there should be mounting bolts to unlatch the entire device.

The lift gate panel is the entire plastic piece covering the lift gate (aka rear hatch, rear door, tailgate). There's a good thread here telling you how to remove it but basically pull the bolt holding the strap and it just pops off.
The plastic you see on the inside is what is used to seal the door. It's attached with a non-permanent adhesive. Just pull it slowly and steadily and it will come off. When you are done you can just put it back into place.

Yes. It is press fitted over the shaft. I had to make a small tool (tiny puller) to remove the thing when I was replacing the arm. Again, it just sits on the shaft, very tight though.
