That all depends on which type of wheel you intend to use. If you intend to use an alloy wheel, which most of are hub centric, you may need an adaptor, depending on how the wheel is designed. If you are going to use steel wheels, most of which are lug centric, all you need is to be sure the wheel fits the hub you intend to use, and have the proper type of lug nuts.
Thanks for the info. My thought is to buy TRD pro wheels to match my Runner, but most importantly is that on the trails I could use them as spares. Also my plan is to use the wheels to rotate onto my Runner. I can't find a hub centric adaptor that meets the bill. I found one on ezAccessory.com, but it as a 14 by 1.5 stud. Factory studs are 12 by 1.5. Its a super light trailer and I thought a normal adaptor from the 5 by 4.5 to 6 by 5.5 with 12mm studs should work..

If the wheels you intend to buy, use the same shoulder style lug nuts as the oem 4Runner alloy wheels, then you will need to make sure the hub on the adapter fits the center hole on the wheel properly, to maintain the hub centric fit. I would contact the manufacturer/distributor for the adaptor you are looking at, to get the exact size of the hub to see if it matches the hole on the wheels. If not, then you will need an hub adaptor ring to make up the difference. That is assuming that the adaptor hub is smaller that the wheel hole and not larger. If it is larger, than you won't be able to use that adaptor without machining the hub to fit the wheels.
I don't have the specific information, but I seem to recall that there are some adaptor manufacturers that may be willing to fabricate the specific adaptor you would need, but I suspect it won't be cheap.
