Well, I was going up and over a dirt mound and my gf noticed that one wheel was pointing a different direction then the other. I hopped out and took a look and at first I though I had snapped the ITR. I tore the boot open and had found that the tire rod had come un threaded from the ball joint to the rack and pinion.
So a little history. About 3 weeks ago I got new tires and sent the car in with my GF to get them mounted, balanced and an alignment. Before they could do the alignment the rack and pinion bushings needed to be replaced. So, now with new tires I took the rig too some back roads and found a little mound I could crawl over. Then thats when the tie rod came undone.
Got it threaded back in and gingerly drove it home. This morning I got to inspecting what had gone wrong and I found the issue.
The locking nut/washer wasnt hammered down to lock it.
This is the side that came un threaded (passanger side)

So my question is; in which service did they need to go into the boot, resulting in the improper locking procedure, which led to this incident?
And what should I go back to the shop who did all this with? A refund of service, new alignment (of course), what repercussions should take place? I am nervous about even going back there, as I do not trust them any more.

I am very glad it cam undone where it did and how it did, I am veryy lucky nothing was damaged, and no one was hurt.
