Intermittent rough idle

Any idea what could cause a very intermittent (couple of times a year maybe) rough idle at startup? This has been going on for years and its not getting any worse so I'm not concerned about it - more curious than anything. It happens on both cold and warm starts and when it does, if I give it gas a few times it slowly gets better until its running normal, then it usually doesn't happen again for a long time.

I'm glad I'm not the only one. Mine is a '95. Don't recall how long its been going on, but its been many years - I got it new. I always forget about, thats why I've never posted, but it happened a couple of nights ago and that reminded me. It always throws me off because its so random. I changed my fuel filter a year ago thinking that may help, but it didn't.
