Last couple days 4R ran rough/missed at start up, then smoothed out after 15 seconds.

coils are common to go out in all the V6's.
Swap the coil with another cylinder and see if the misfire moves with it.

so what happened?
I am curious to find out what happened when you moved the coils around. I have an 04 SR5 and I just had a coil go bad. Took it to mechanic because I was in a hurry to get it fixed and they charged me $300 to change one coil. Drove it away from mechanic and same problem occurred. Took it back to the mechanic and the found the gap in spark plug was .80 (pretty far from .44 required) they re-gapped it to specs and it is running fine. I am thinking about replacing the coils when I change out spark plugs.

I have had no issues since I ran the fuel injector cleaner through the truck.
Of course, now that I wrote this it probably won't start this AM!
Why are you going to replace all the coils? I would also request my money and old coil back from that shop.

If the problem comes back do a block leak test to check for exhaust in the coolant. A small leak in the head gasket is fairly common on the 4th or 6th cylinder on these engines after 120k. If it starts up and idles rough for a few seconds it could be due to a small amount of water leaking into the cylinder. The problem can be intermittent since the piston can rest at different positions in the cylinder when you turn off your car, leaving more or less room for water to leak in. It wouldn't hurt to do this test anyways since it is really cheap to buy the kit from an auto parts store. Plus if you do have a leak you could catch it soon enough before the block or head or both warp. Obviously its worse case scenario, but it wouldn't hurt to check. I just went through this on my 05 runner. I had an intermittent misfire/ rough idle for about 5-10 seconds on start up after leaving it overnight. I had no other signs or loss of performance. Ended up having a warped block and got a low mileage used engine to throw in.

I was thinking about replacing the coils because I had the one go bad and my SR5 has 183000 miles. and I figured if one goes bad the others are soon to follow. The mechanic I went to said the he switched the bad coil #5 with one of the other coils #3 and the reading came out as a bad coil in #3. So I don't think that I have any grounds to get my money back. Although, when I look at the sensors on the engine, it only looks like #5 has been touched.

You can do it! I followed these instructions for changing the spark plugs in my '04 4R and it took me less than an hour and a half. From start to finish.

I will tell you one thing that helped me tremendously was a magnetic socket wrench that I found at Advance auto parts. It was amazing. Very strong and I didn't drop a thing.

The one thing I was surprised about and maybe someone can help me out with this, is that my gas mileage has actually gone down. When I previously filled my gas tank, it would tell me I had around 405 miles (actually got about 380 miles) now it says I am gonna get about 390 and I actually get about 360 miles. Can anyone please explain this to me?

My money is on a blown head gasket. Did you ever have the block leak test done or did you just move the coils around?
