Purely speculation, but if it was cold, just started up and was driving, then blew, I would suspect head gasket leaked coolant into the cylinder and bent the rod on startup, got enough momentum going driving down the street to fail. It's not common, but happens.
OP, what does the coolant look like? I mean, not that it matters ultimately, but just curious. Is it low? Foamy?

So, it puked oil, but still shows oil on the dipstick?

JDM's are more work to put in too. You have to swap a lot of parts over, a US engine is drop in & go.

Supposedly from times way back, the yota lore on YT was that there were two factories in Japan that built the 5vz. The rod through the block engines were made in just one factory and one could discern if one had a possible imploding engine by checking the VIN number. Can't say if I put much merit in this though if the builder with that torque wrench was not torquing them correctly, then it may be a possibility.
Went to see a 4runner for sale for parts in DC back in 2011, hole in the block. The guy didn't even get out the driveway. 126k if I remember correctly. No warning and the internals were spotless.

I should be back up and running today. They resealed everything on the new (used) motor and had it in and running last night. They wanted to run it through a couple of hot cycles with an oil/detergent mix, check for leaks, change the oil, a couple more hot cycles today with oil, and check for leaks again. If everything goes according to plan, I will have it back on the road tonight.
Always risky buying a used motor.

It will be close to $5k when it's said and done. The used 88k mile USDM Motor was 1800.

That's a little steep and about right if you don't do it yourself or have a friend. I had my 3RZ rebuilt, new heads, engine bearings, bored out, new pistons etc for $5000 by a friend with a shop. He had to send it out for machining but he put it back together. Would have been cheaper if I bought a used motor but I knew my block was good. Good luck with the used motor.
