Did you buy your truck from a dealer or private party? Either way, you can check with seller. If it was a dealer, you can probably get it from them at no charge, as this is standard equipment.
If that's not an option, there are several for sale on ebay.

Above is the link to how I had to put a set together. Depending on if you have a scissor jack or a vertical piston jack, you may be able to find a decent one at an auto parts store.
