New brakes, old squeak

So when I bought my 4Runner, it had a squeak to the brakes in the same spot every rotation after about a month. Eventually got to the point where I couldn't take it anymore and took it in for new brakes thinking pads were done for. Took everything off, and the rotors were actually being gouged as if there was a metal scrap inside the brake and had chunks taken out of them. Replaced them right around beginning of September, brand new pads AND rotors... 2 months down the road and just yesterday both rear brakes are beginning to do it again, passenger when I lightly begin to brake and then driver as I start to push harder and come to a stop. Can anyone think of a cause for this? I will already be returning to the shop and seeing if they can either fix or replace them.:wtf:

My 03 has had squeaky brakes from day 1. Pads, rotors, new calipers, grease...none of it made a difference. It's not bad squealing but its there at certain pedal pressures. I have learned to live with it.
 
Cheap parts can also cause squeaks. Same with old brake hardware.
Also make sure all metal contact points and shims are lubed.

Unfortunately mine is a very shrill grinding, like nails on a chalkboard. I am going to have to do something because I will never be able to live with it lol.

It could be that the hardware is old, besides rotors and pads everything under the truck is probably original so 192k miles. Will have to double check on everything being lubed down there when I get a chance to look at it. Thanks!
