I'd just go with the OEM Denso or NGK. The 4th gen 4Runner typically comes with both brands installed from the factory. NGKs on the left side of the engine and Densos on the right side of the engine.

Go with all Densos, they wear better.

No one knows. Toyota has been doing this for a long time. If you search the threads, you will find this issue already discussed under the maintenance section.
