The fridge slide is now currently listed on their site so you will be able to add it to your cart (and purchase it separately). This means you will not have to perform steps 5 & 6 above for the fridge slide, nor will you need to call in to order it if you are not purchasing it with a fridge.

If anyone is interested, I just spoke with SquareTrade warranty company and they will offer warranties on these. It would be $119 for 5 years on the 40 and 50 liter units ($60 for 3 years) and $99 for 5 years on the 30 liter unit ($50 for 3 years).
