Opinions on this knock.

So on my other project I just put fresh 6 quarts of full synthetic 5w30 and started her up. This truck has been sitting since 2018. I revved it lightly a couple times at operating temp and got a horrible knock.

I already feel like I'm going to have to pull the engine but opinions are welcome. Not sure where to go from here just yet, but it sounds pretty grim.

That doesn't sound good for sure. Did it run fine when it was parked?

Pull the dipstick and use a flashlight to see if there is any sparkles in the oil. It kind of sounded like the major knocking occurred on the decel, could be a rod bearing(s) going bad.

You could also drain the oil and cut open the filter to see if there is metal in there. If there is, plan on rebuilding or replacing that engine.

What does your harmonic balancer look like when it starts making the noise? Flex plate is another one to look at. It does sound like your typical rod knock. Sounds more like something out of balance. I would also look at the timing belt and tensioner.

I have never heard an engine go from normal to heavy knock within seconds like this one did! I pulled the stick and didnt see anything sparkly at all. The oil looks clear as day, its almost hard to see it on the dipstick. I know that noise means I should pull the engine but... if it is rod bearings do you think this is something that is worth attempting from under the car? I only ask because I'm a hobbyist at most, I'll do a full rebuild or put another motor in it if I have to but I prefer not to.. (said everyone ever probably)

What do you mean what did it look like? the engine cant get much worse so I can take a video of the engine bay with it running if that helps figure anything out.

The balancer can come apart and start making knocking noises. You'll see it shaking. If not then I've seen a timing belt tensioner go out and cause a rod knock noise on a 2UZ. It was original to the engine too.

Does it have oil pressure? Or oil in the crankcase??... Good call on shutting it down. I had 5VZ-FE sit for many years not started and fire right up with no noises. That's definitely not a good sound at all..
