P0300 + Running rich and rough advice

So my 06 Sport V6 4WD has 93k miles on it and thankfully never suffered a breakdown. A buddy borrowed it today because he was worried about driving in snow (the main roads are clear, just neighborhoods that have a little dusting on them).

Well, I got the call maybe 30 minutes later. The CEL is blinking, which disabled VSC and Traction control (and also shows the slipping car logo). He said it is running very rough, but drivable under 40 and heading back.

Given its 9 years old and has mostly original parts, I replaced the gas cap (just filled it up yesterday). I connected the scanner and is say P0300 only. I didn't clear the code and decided to drive it around the neighbor to see what's up myself. First, it barely started. Was trying and trying for nearly 10-15 seconds before it turned over. It was very, very rough and nearly stalled out. I was able to drive it for about 3 minutes and noticed it is spewing white smoke out the tailpipe. Pulled it back in the garage and I could smell it was very rich. Looked at the gauge and it used half a tank to go 57 miles since I filled it up last night.

I picked up a new gas cap from the dealership and tried replacing it. It made no difference, but I didn't reset the code or pull the battery for a few. I figured even if the code stayed in memory, the car wouldn't be rough if this fixed it.

I can't remember when the spark plugs were last replaced (if ever), so my next thought is to get those replaced. Any thoughts/advice from you all though?

Basically the ECM says, "HI! One of my cylinders is misfiring, but I can't be arsed to figure out which one, GOOD LUCK! :D "
A gas cap will not fix that (why do people think gas caps magically fix vehicles?) if the MIL is flashing that is a "Catalyst Damaging Misfire" indicator meaning you're supposed to turn the vehicle off.

Does your scantool do datalist and/or freeze frame data?

Yes, it can read the DTC codes. What I am not sure about is if it is just listing everything and whether there is a value, or each item listed is a code the car is reporting. There were dozens of sotred DTC code turning the switch to ON without trying to start the car.

Wow, were you serious about using a half tank of gas to go 57 miles? If so, I imagine your cats and probably O2 sensors are now toast. Your friend wouldn't have any problems driving in the snow because the underside of your 4Runner would have been red hot and melted any snow within 10 feet!
Blackworks is the professional, but I imagine you've got at least one (maybe more) injectors stuck open to burn through that much fuel in such a short distance. I'm surprised you don't have more codes related to rich fuel conditions.

I'm serious. I'm roughly 45% of the tank left and I reset Trip A when I filled it up before the storm. 57.3 miles to go from past "F" to about two needle-thicknesses below half.

Crossing my fingers is sounds worse than it is though. The next step from what I gathered is to check/replace the spark plugs. Interestingly enough, Toyota was the cheapest of everywhere I called to get that done. It got towed there a few hours ago. The service advisor tried starting the car up and it turned right over without any hesitation or complaints. They had a line of cars to work on, so I probably won't hear anything back until tomorrow.

Yeah, the injector for #6 is wide open, which fouled up the spark plug. They are quoting $850 for the repair. :O

What makes them think it's "Wide Open?" Also WHY is it Wide Open? There's a couple reasons that come to mind on why an injector is stuck open.

Now that I think of it, I don't recall of they said wide open or stuck open. But the advisor said they tested the fuel pump and line in case I had a leak given the heavy fuel burn. He did they feel the catalytic converters appear to be in good shape even with all the fuel going through. He said the gas smell coming from the car is a result of this.
 
how sure are they? what if it's actually not the problem?

I will ask that in the morning. I'm worrying that I will have to have them do something though as the car stayed there overnight now and they did their diagnostics, so I'm sure I'm going to get charged for something if I don't have them do a repair.
 
They were quite sure of the repair and I proceeded. Got the call this morning that it went well, the car is running smooth, the catalytic converters are in good shape, but it is throwing a code for the O2 sensor. That will be another $200, but I should be done then.

What was the repair, replacement of the injector? Which O2 failed?

Replacement of the injector on #6, replacement of all the spark plugs, replacement of one O2 sensor.

I do not know which O2 sensor failed. I was on a conference call at work and listed to the voicemail and emailed back to proceed with replacing the O2 sensor for another $199.

I am happy to report that after a week and 100 miles of driving, all seems to be well and the the truck feels like it's normal self. Purrs like a kitten at 6-700 rpm and is as smooth as ever. Thank you for the feedback everyone!
