The advancement of modern technology and medicine has allowed progress in the better care of the patient, decreasing risks and maximizing benefit. It's all about risk vs benefit. The risk is that this guy could bleed out, or loose his leg. The benefit is saving a guessamated $10,000 of which his insurance would have helped him out. Based on the information presented, yes I would pay the money to minimize risk of dying or loosing my leg.
After you have seen several people have multiple surgeries after an accident to save their life and limb, the sequelae, and the extensive recovery it takes, trust me, you probably would take the helicopter ride.
Your question is like asking "Well in 1800, what did they do for a brain aneurysm?" The answer is nothing. Now, it is a very risky surgery to repair it, which could possibly result in death. The procedure has saved countless lives.
All I was trying to say is that from the standpoint of medicine, declining a helicopter ride due to cost (benefit of saving $) does not outweigh the potential risks associated with bleeding out or loosing a limb.

I'd personally be more concerned over liability. Even if he was cleared by the professionals and gave his verbal consent, you'd be in for it if he decided that somewhere along the way to the hospital his leg got hurt even more. Then you'd have a lawsuit on your hands, just for trying to be nice. Or, he could have caused a commotion and tried to sue you, just to pay his hospital bills.
People suck sometimes. Unfortunate, but true. You would have looked like an ass, but you should have gotten a signed waiver from him, even if just on a spare napkin.
This is the kind of stuff that makes me sick. Alex did a nice thing by doing this, he shouldn't have to worry about being sued by the guy and what not. I hate the sue-happy attitude that this country has adopted.
Maybe we can get this thread back on topic with people doing good stuff for other people in their 4Runners now. I don't have any pictures but I've jumped many people with mine, and am always willing to help someone get unstuck if needed.

For what it's worth- I am done helping people stuck on the beach. This past summer I pulled out an Expedition who had buried himself to the frame while trying to pull out his friend's Mercedes S-Class. With the Expedition out of the way I went to get the Mercedes. I literally drug his car through the sand like a sled, and his front bumper got pretty chewed up. He started talking big, so I nicely let him know that I appreciated his help, and reversed out of his sight.
There are perks to not having a front license plate.
Some sort of release of liability form would be cool to have for stuff like that.

You are correct, Alex DID do a great service and he SHOULD NOT have to worry about liability, but unfortunately, this is the world we live in now. Sue happy greedy people. Those people are why our insurance is so high. This is a great deed and it all turned out well, but sometimes, you just dont know. I let my 38 year old neighbor ride my go kart one night...he flipped it, and...you guessed it...is sueing me for his hospital bills...after I drove him there so he wouldnt have to wake his wife and kids.
