I'll get you a shipping estimate via pm and yes it comes with brackets. I only have two of the 4 mounting bolts but I ran it that way and it was always fine. It actually made it more resistant to bending because if you got up against something it could rotate up.

It mounts to the sides of the tie down hook area, just below the front of the frame rails. There are threaded bolt holes already in place on my '04. No idea about an '01, but I wouldn't be surprised if it would fit. I've attached an image of the area. The width from bracket to bracket is 34.5 inches exactly.

You're right. The bolts I have go in the bottom threaded holes. The top holes are not threaded but they do go all the way through. You would just need to get to a hardware store for bolts long enough to go all the way through, and then use nuts on the back of them. Recommend lock washers as well obviously.
