Steering wheel shake/vibration

On my 04 I feel every bump in the road and vibration through the steering wheel. It drives me insane.

I have bought new tires (needed them) and balanced them, changed the brakes and had the 4runner aligned.

Does anyone have any idea what to do next? Its getting to the point that I dont want to drive it much.

ball joints? sway bar bushings? just a couple of ideas.

MY 05 4R vibrates insane as soon as I cross 60mph till all the way to 70/hr and then I dont feel any vibration after that. I also it tends to wander around while driving and I have to keep the steering steady. I have Bridgstone HT (not new) I still have to figure out the root cause.

You may have thrown a wheel weight or need a new tire. I only have 5500 miles on my BFG's and had them rotated last week. When I left I had a bad shim and took it back up to 4wheel parts. The tried to balance it and had to order a new tire for me.

When I come to think of it, I think it started happening after I put 1.5" spacer which I got off on ebay. Do you think spacer is faulty? or some wheel weight came off while installing the space?
I think I would try taking the spacer off to see if shake is gone, of not will rebalance and realighn.
