Suggestions?

I have a 95 4runner, automatic, 4WD, 3.0l v6. Lately I have been having trouble starting but couldn't get it to start. I know its not the battery or starter because it turns over but then it tries to idle for about a split second then the engine dies. I finally got it running by turning the key with my foot slightly on the gas. when the engine started i kept the rpm's up manually with the accelerator for about 5 minutes, then slowly released the gas pedal and the engine idled by itself. It doesn't do this all the time but seems to do it more when it is cold.. About 3 weeks ago I replaced the alternator, did a small tune up... Any suggestions?

Well check the pressure on the fuel rail before condemning the fuel pump.
There always the fuel pressure regulator or the fuel pulse damper that could be a problem.
 
I am curious because I am getting a continuously blinking OBD1 code, which indicates nothing is wrong... It has been doing it for about 3 weeks now but today is the first time it actually would not start, we did however make it home after we got it started.

Sounds like a sensor to me. They sometimes get finicky in the cold.

No smoke on start-up is there?

What type of spark plugs did you use? Try NGK coppers first. I would also change out the fuel filter and check the pressures(rest and running) to see if the pump/regulator are faulty. It might be a good idea to check under the distributor cap and see what the contacts look like, clean the inside a bit. Vac leaks are another that can cause starting issues, you check around the injectors first. It's also worth cleaning out the throttle body, this will improve metering. If it's never been done, I would change out the EFI relay($12 at the dealer). These can cause a "crank, no start" condition when they go. These do go, our dealer actually stocks them on the shelf( and they stock virtually nothing).
