Suddenly in cold weather starts, when I shift to drive and step on the gas my 1999 4runner does not excelerate, instead it crawls forward while the gas revs. Does anyone know what the issue may be?

Do you know the condition of the transmission fluid? I would look at that first. Maybe the cold is making it less viscous (prior to warm up) and that is affecting the torque converter or the tranny components.

I had fluids checked by Toyota specialist, who sent me home with a clean bill of health

That's all this web site has to offer is check the transmission fuild! Certainly there is more knowledge out there. I have found 3 other similar questions, all without insightful responses.

Hmm, maybe that means no one else has had this issue? I faced the same thing with my transmission issue. Some suggestions, but nothing concrete.

It all starts with the fluid. If Toyota says it's OK, it might be. Have an independent transmission shop check it.

There are shift valves and solenoids that might be sticking. Sounds like you aren't getting enough line pressure to flick the valve or solenoid all the way over. Does it 'bump' into gear and go eventually after revving for a while?

Ok when I just had all the service done on mine he dropped the pan. The fluid was fine but there was still build up that was sometimes causing issues. I had 191k on my truck, he dropped the pan and replaced the filter and has me scheduled to come in with my next oil change to do it again. He did not and will not do a power flush since it can cause more problems, that is why I am going back in for a second service in case more junk gets unclogged. My main issue was a slipping between 3-4 and lazy lock-up. However I did off-road and was on a grade pushed the go pedal and nothing happened, then went after a little bit.

It will easily go in reverse, which I believe tells me the pressure is fine. When I slowly rev it and crawl, it eventually goes without a bump. I've had 3 different 4runners with over 100k miles. All had an occasional wump sound when moving from nuetral to 1st gear on cold starts. Today I heard that wump just as it started moving normally.

Pressure is fine, probably. Could still be sticky valves or a shift solenoid.
