1993 3VZE and transmission problems?

Hey guys, Im new to Toyotas and new to this forum. I love my new/old 4Runner, and this forum is great!

But... I recent have been finding problems with my old runner. She has 198,000 on her and runs pretty good for that. And i have done recent routine maintence on it to make sure to get it running perfect.

Maintence I have done so far:

Fresh oil change and fliter
brand new fuel filter
new plugs, wires, and rotor cap
brand new cat

Now the problems I am having:

Rough Idle now and then, transmission shudder at 55mph at 2000rpm.
exhaust leak from somewhere other than my cat?? And when I remove my gas cap it has alot of pressure coming out. (i remove it everyday because it worries me).

Now I have searched and searched through this forum and yotatech forum and toyotanation forum. No answers, so I came to the best! Any advice would help me out!

When you remove the gas cap, are you sure the noise isn't from the tank sucking air in? Because that would be normal.

If you have extreme temp changes throughout the day, pressure can build up in a gas tank. I've had gas leak out of the fuel neck with a full tank of gas, back when I had a '69 Falcon. I think I even had it happen with an old 80's celica I had.

The gas cap sound is like when you run your tank almost completely empty.. and it is always hard to twist the cap off.. I think the fuel pressure regulator is bad and I bypassed it with a screw in the vaccum line.

I fixed the exhaust leak earlier today also.. found loose bolts on the manifold.
