Hey all,
Hoping I can get some assistance with this issue, it's not a 4Runner but I think same troubleshooting steps are in order, regardless. I posted this on rav4world last week but have not gotten any replies. Thought I'd give it a shot here:
Original Post Last Week:
2011 Rav4 v6 w/ 107K miles - VSC, CEL, 4WD lights on, code reader indicates:
P2196 Heated Oxygen Sensor 1, bank 1 - signal stuck rich
P2239 Heated Oxygen Sensor 1, bank 1 - positive current control - circuit high
Replaced the O2 sensor (Air/Fuel) on the exhaust manifold (plug end wires were all frayed somehow) and cleared the codes. Drove a few miles and dash lights came back on with:
P2195 P2196 Heated Oxygen Sensor 1, bank 1 - signal stuck lean
Question 1: Did I replace the correct sensor?
Question 2: Did I replace it with the right part?
I bought all four O2 sensors (Denso) on Amazon. Here are the part numbers:
Denso 234-9022 Air Fuel Sensor - This is the one I used to replace S1B1 with
Denso 234-9049 Air Fuel Sensor
Denso 234-4501 Oxygen Sensor
Denso 234-4261 Oxygen Sensor
I've since cleared the code again and will go for another drive soon.
Any help is much appreciated!

I tested the B1S1 air/fuel sensor that was frayed, after making sure none of the exposed wires were touching each other and the sensor tests good. I decided to keep the new one, in case the old one malfunctions in the future. I then replaced the B1S2 air/fuel sensor and cleared the codes. I then scanned for codes and immediately got P0171 - System Too Lean Bank 1. Assuming it was a vacuum leak, I made sure all the hoses were good and tight and cleared codes again. I then drove around for a few miles and the original lights came back on - rescanned and it's back to P2195 Heated Oxygen Sensor 1, bank 1 - signal stuck lean.
I went ahead and identified the location of the other two sensors and tested both before removing them and they tested good on ohm meter, within bounds. I then took out B2S1 and compared it to Denso 234-4261 I bought on Amazon, and they look different.
The new one has lots of holes on the body of the sensor but none in the front:
The one that came out of the vehicle only has a single row of holes on the body of the sensor as well as 3 holes in the front. The Toyota OEM P/N is 89465-0R020. I double checked on few different sites and it says Denso 234-4261 is compatible with 2011 3.5L v6 Rav4 but it doesn't look the same.
I put the old one back, since it tested good.
At this point I'm not sure what the problem is with B1S1 but I replaced both air/fuel sensors and I'm still getting the codes.
