If you look at the parts diagrams and body lift kit pictures in various threads. You'll see that for the most part the top bushing has a metal washer bonded to the top, while the bottom bushing just uses a washer to retain it. The sleeve slips through the center to prevent it from being over tightened, while the two halves pinch the frame in a way that isolates it.

I think when I get more time on my hands I will try my hand at making a few custom body mounts.
 
You can make a quick test with a steel sleeve and some 3M Window Weld. Pretty close to an off the shelf bushing, around 60 A or so.
