5100 problem

I installed 5100's with moog fj springs recently. I used kyb strut mounts. I developed a loose squeak/clunk sound. I found both top bushings loose. The drivers side is worse than the passenger. What would cause this? I can see that both the shock boots have dropped too. Did the strut mounts fail? The top nuts are tightened down all the way. I can see there are no more threads left for the nut to go down anymore. Thanks in advance.

There should be a bushing that went under the top plate that looks similar to the one you have on top. It looks like whoever assembled your struts may have forgotten to put it on. This is the order the bushings and washers should be in. The top plate goes in the middle.

EDIT: yep, like morphious said, you need to relocate that washer you have on the bottom on your upper bushing to the bottom of your lower bushing.

The person that installed it forgot to install the bottom washer. The top of the shock has now pushed through the bushing and is bouncing off the bottom of your top hats. The proper installation (from bottom up) should be shock strut top nut, washer, bushing, top hat, bushing, washer, nut.
 
That would have been my mistake as I did the install. Is there supposed to be two bushings? I did not have anything left over. Can I just but new mounts and bushing and be set?

In your last pic it looks like you have a bushing under the top plate. Just need to put that washer down there.
