I've seen 1/8" or even 1/16" before.
You got 58,000 miles on one set of brake pads...that' well over expectation.
You're at 1/4" & it's squeaking, maybe the wear indicator is scratching the rotor already.
It's cheaper to replace the brake pads then to replace the brake rotors, plus, you'll feel safer knowing that the brake pads are new.
I had Honda Accord, Honda Civic & Toyota Camry...and replaced the brake pads around 30,000

About 3k miles /Aug08 ago the dealer said i had about 3mm left. I ran about another 1k miles then replaced them.
According to the Toyota online service manual on the 2k3, minimum allowed pad thickness is 1.0 mm. I personally don't plan to go quite that thin, probably in the 2mm range is when it's time to change.
I have a PDF that's labeled "Lexus Technical Training" that provides a disc brake overview. Unfortunately, I don't remember where I got it from, but I would assume I saved it because it was supposed to be related to the 4Runner/GX. It has a diagram that suggests the wear indicator will trigger at 2.5mm, but that's possibly a Lexus specific spec.
1/4" is roughly 6mm with the original pad thickness being 11.5mm, so you're about half way through.
From time to time, you can pick up noise from uneven distribution of materials in the pad. It'll usually wear away with use, happened to me couple of years back. If you've really got 1/4" on the pads and they're wearing evenly, I'd just keep driving.
