Do these temps seem normal?

The tranny cooler would be an added benefit for the trans. Cooler temps usually equal longer life. The B&M 70264 is a popular choice. It has a low pressure drop (doesn't begin opening until the fluid is 170F, where at 180f it is open all the way) and fits nicely in front of the cooling fan.

The real issue that needs addressing is why the voltage is below 13.2. How old is the battery?

The battery is close to two years old. Never had any issues with it. It's an interstate battery.
 
Drove around town for about 30 minutes in 99 degree heat. Stop and go traffic.

Your truck is 'fine' with all those numbers. Improvements can be made with engine temp, trans temp and battery voltage - but none will stop you from driving the truck 10's of thousands of miles!

Voltage is as reported by the ECU, which means that's the voltage it senses is running the injectors and coils (used for adjusting injector opening times, etc.).

Low voltage is not caused by the battery. Might be weak ground/power wiring or a bad/weak alternator. It isn't fatal unless you don't see >13.0V in normal driving (engine above 1k rpm, say).

Charlie is correct. I should have clarified to check your battery connections first.
