I have 230k on my 94. It's the v6, head gasket replaced at 80k. The 5speed trans has some issues, possibly the syncros are goin bad. It always grinds goin into 3rd and I have to apply extra force to get it into 2nd. I just wanted to know which would be more cost effective and get me the most performance.

The most cost effective move would probably be to grab a junkyard trans and throw it in. The R series trans aren't known for having much trouble, so you'd probably be ok. There's not really any such thing as a performance transmission for these trucks though, they are what they are.
If you want more oomph at the pedal, research a motor swap. The 3.4 is a common swap for your truck, and a good upgrade.
