idle problems

Hi,
I have a 2005 V6 4Runner with 240,000 highway miles. It's been a flawless vehicle. Lately it's been idling roughly for the first 10-20 seconds on some cold starts. It showed a misfire on cylinder 4 so I changed the plugs and cleaned the throttle body but it didn't fix the problem. Now it will idle roughly at 500rpm in gear with my foot on the brake. I'm at a loss. Any suggestions??

Have you done any other maintenance like oil change? This will sound weird, but whenever I reset the ECU (unplug battery for a min or so) my T4R will do the same thing. when i'm sitting at red lights, it will idle rough about 500. But whats also weird is it fixes itself as if the ECU is "learning" all over again. It always fixes itself after a day or two.

It sounds like a sensor is bad.
 
My dad's an old school car guy and the head gasket was the first thing he mentioned as well. I'll look into it. Thanks!
 
I am agree with Mr. Panic. Few weeks ago after disconnect my baterry for terminals cleaning and oil change, when i started the engine i noted the idling was 400 rpm!!!! i felt vibrations in the engine and sometimes at the red ligth it almost turned off. After drive for 40 min idling normalized itself and went up to 700 rpm. Do not concern too much about it just drive for couple of days and it will dissapear.

Thx for the messages. The coolant level was dead on full so a head gasket isn't likely. I'd like to fix it myself to save hundreds getting the dealership to look into it.
