I'll chime in with what I have experienced. Bought the Pedal Commander back during Covid in 2020. I understood that it doesn't make the car faster. Just adjusts how fast you hit full throttle sooner on the gas pedal scale.
I have simply focused on two of the settings:
1) City - no other adjustments + or - on the scale.
works great for city and highway driving...for me. Quicker response on the gas pedal gets you up to traffic cruising speeds right away.
2) Eco - again no other adjustments + or - on the scale.
This one has the best benefit for me going off-road. With a slower increment on the gas pedal it's perfect for just getting that right amount of wheel spin without over-spinning wheels right away. Hidden benefit is that I set the pedal to Eco when teaching my daughters how to drive. That way they would not over-press on the gas and have the car lurch forward uncontrollably.
Dangerous part of Eco is when trying to drive on the highway. It's too slow to overtake slower cars on the highway safely and I find I would have to practically floor it just to maintain 65mph speeds and forget about it if you are going up a hill. I did not feel safe to be in Eco on the highway at highway speeds.
It certainly has it's place and I really like it. One just has to keep in mind the reality of it and that it is that it doesn't make your car faster it simply speeds up throttle response. For example in City/Sport/Sport+ mode you hit full open throttle before the pedal hits the floor. Pushing it to the floor doesn't give you more open throttle, you just get there quicker based on the mode.
For comparison whenever I get into my wife's Honda Pilot I feel like I have to mash the gas pedal just to get it off the traffic stop. Not the same with the Pedal Commander on the 4Runner. As soon as you touch the gas pedal it starts to react in City mode and higher.
I tried Eco mode with local traffic driving but not long enough to see if it would improve mileage. Eco mode does have its place too if in heavy, stop-and-go traffic. Keeps one from over-running the car in front before having to hit the brakes again.
All-in-all I am very pleased with it for what it does. Turns out they just put one out for the Pilot so I am considering getting one for that car too.
