I have a 2006 4runner v8. Lately it has been running louder than normal. I feel like it needs to shift but the RPMs are usually somewhere around 2 so it seems to be shifting fine. It sounds like it is always struggling to make it up a hill, even when moving slowly.

Any thoughts?

03 V8 106K same here.. i already cleaned the air filter (K&N drop-in) and lubed the prop. shaft, TimeBlt/WaterPmp, and new calipers and brake pads. It's strange the feeling because I am getting 18.0MPG and she feels weak (I do live at Colorado Springs Elevation) she is strong when I really get on it but she feels like she's just dragging but maybe not eh? with the 18MPG and all:iagree::iagree::iagree:
 
If you can, drive on the highway for a little while 30 minutes to an hour if possible. It will feel a lot smoother when driving around town.
 
When you say it sounds louder, do you mean it sounds like your cooling fan is locked up and not allowed to slow down at cruising speed? With your car off open your hood and see if your cooling fan will free spin or if it is really hard to turn. If the fan clutch wont let the fan slow down then it can sound loud like the vehicle is struggling.

Some thought maybe something to do with the exhaust? Or maybe not all spark plugs are firing?

I have a 2006 4Runner Limited V8 with 125,000 miles on it. It is doing something similar to the problem you have. It started running loud about two months ago. It used to do it intermittently but now it is doing it usually after I have been driving for about 30 minutes or when I am running errands, so stopping and starting the engine several times. It sounds like it can't kick into gear, and it struggles to accelerate. Does anyone know what the problem could be?
 
My truck does something similar, usually the first start of the day OR if I started it and stopped it, it will do something similar when I accelerate for the first time. My '03 did the same thing though....
 
I too have been having this noise problem on my 06 V8 sr5 and I though maybe it was the heat, where I live in New Mexico it's been hitting 102° on the regular. It almost sounds like a cold start. When I checked fluids and belts everything was fine though. Now I'll check the fan clutch.
 
A few years back one of my coilpacks was full of water. Engine was probably running on 6 cylinders. It was such a gradual failure... to be honest, I didn't really notice.

After the shop fixed it, it was like a whole new vehicle!

I tend to be much more sensitive about strange noises and weird behaviours now. If you think there's something wrong there's a solid chance there is.

It'd certainly be worth checking out the ignition system and perhaps verify the cats aren't plugged up or something like that.
