One wheel is cooler than other three.

After a few minutes stop and go local driving I put my hand near the wheels. The front right one felt cooler than the other three. When I sprayed some water on the brake discs, the other three went sizzling while I can still touch the front right one with my hand briefly.

Other than this, the 4runner drive fine, braking is smooth and no leaning on one side etc.

So is this normal? The 4runner went through state inspection about 2 months ago. Would they check for seized caliper and such?

Seized calipers wouldn't be apparent to a tech when doing a state inspection, as long as it appears you have brakes it would pass inspection.

That being said it sounds like you do indeed have seized pistons on the three usually hot brake assemblies, you should be able to touch them briefly without harm after normal driving and braking, if they are sizzling when you spray water on them they are getting too hot.

I would also think the other right front brake isn't far behind the others, so I would replace them all.

i would do a brake fluid flush and check for frozen calipers.

He stated that he could touch the cooler one "briefly" so I'm assuming that means it was still sorta very warm to hot so I don't think it was seized open.
