I bought my '08 V6 RWD with 99,465 on the clock - at that time a full 100k service was done. Since that day the oil has been changed every 5k, new battery, mild lift and new wheels and tires. Today I am nearing 135,000 and need to know;
A) When should the next full service occur?
B) What should/should not be done during that service?
C) Is there any benefit to switching to "high mileage" or synthetic oils, or anything else to prolong my journey with this truck?

I'm nearing 150k and the maintenance book doesn't go that far. But some common sense stuff I'll be doing:

*Stock V6 plugs are copper. They're rated for 30k miles
*Brake fluid flush (every 2 ish years)
*Drain and fill ATF (every 30kish with Maxlife). Use excess ATF for PS flush.
*Diffs and transfer case (every 50-60kish)
*Alignment check (or visual check of tire wear)
*Grease drive line every oil change
*Lube up brake parts 1x year (usually before winter hits)
*Visual check of boots, ball joints, linkages, Xreas, and other odd rust spots.

I find that the higher the mileage climbs, the more these little checks and maintenance will extend the longevity of the truck.

I run Mobil1 syn + rotating filter of choice for 7.5k ish miles. I wouldn't go to "high mileage" formulations unless you notice oil consumption or leaks. There's a theory that switching to synthetic at later on in life/higher mileage may cause leaks as it cleans the motor of what ever sludge was sealing cracks. You may want to try synthetic blend for an oil change or two to see if leaks do occur before going full syn.

What maintenance was done to your 4Runner at 100k? The 100k maintenance in the maintenance manual is a pretty short list.

The maintenance manual calls for:
-replace engine coolant @ 100k
-rear diff. oil every 30k (while towing), [my opinion is it should be changed at some point prior to 100k]
-replace ATF every 60k (while towing), [my opinion is w/ lift and larger tires, T4Rs fall into this category]

However, the TSB says not to change ATF for life of the vehicle.

Things not covered in maintenance manual:
-lubricate all locks and hinges with white lithium grease spray
-lubricate/clean sunroof tracks
-replace PCV valve
-Power steering fluid exchange
-Brake fluid exchange
-lubricate steering intermediate shaft's u-joint w/ white lithium grease spray
-gas cap, (inspect every 30k) [IMO replace if not done at some point prior to 100k]

Typical maintenance items:
-cabin air filter every 30k. Its behind the glovebox.
-lube drive shaft every 30k
-replace engine air filter every 30k
-replace spark plugs every 30k [I run them longer]
-brake pads inspection
-drive belt inspection
