Rear engine coolant crossover

I apparently have a coolant seeping gasket at the rear coolant crossover.

I am having a hard time find the gasket part number after hours of searching. Though I will admit I'm not very good at this thing.

Can anybody help me with this? Also any idea how hard it is to replace?
