Steering Wheel Shake when Braking

I have been experiencing a steering wheel shake when I brake. It started a couple of months ago and seems to come and go at random. I have a 2013 w about 26K miles on it.

Any ideas if this is a problem with the 4runner?

Possibilities

Alignment out of whack
Warped rotor
Uneven brake pad wear
Seized piston in your caliper
Ball joint blown/bad
Tie-rod bent
Etc..
